Substitute Decision-Making
Part of the Advance Care Planning process is identifying your
substitute decision maker. This is the person or people who are legally
allowed to make healthcare decisions for you if you are not capable of
making the decision yourself. Some examples of these times are:
- Severe illness when you can’t communicate
- Advanced dementia
- Being unconscious during surgery
